Your First Visit

In your first visit, we’ll spend real time with you so we can understand your needs and goals. You’ll meet your whole care team and get set up with all the appropriate technology – a continuous glucose monitor and apps. Because we don’t want to rush your care, this visit will be two hours, which you can do all at once or break into two separate pieces, whichever works better for you.

Please make sure to complete the following prior to your first appointment:

  • Patient Questionnaires (via email)
  • Forms for Signature (via DocuSign)
  • Libre or Dexcom portal invite (for patients who have a continuous glucose monitor prior to their first visit) 

In order to best serve you during your appointment, please bring the following to your first appointment:

  • List of current medications
  • Login information for continuous glucose monitor portal and iPhone/Android app store
  • Insurance card(s)
  • Credit or debit card for any visit copay – we do not accept checks or cash at this time

Please arrive 10 – 15 minutes before your scheduled appointment to allow us to check you in. If you are unable to make it to your appointment, please call or email our office at least 24 hours prior to your scheduled appointment so that we may attempt to schedule another patient at that time. Please understand that if you are more than 15 minutes late, we will need to reschedule your appointment so that your care is not rushed.
