For both traditional visits and care management services, Ryse is committed to providing easy access to the care you need while avoiding unnecessary services. In any given month, you may receive care as a traditional visit, care management services, both, or neither depending on your needs.

At Ryse Health, we believe in making diabetes management more affordable. That’s why we are in-network with most major insurance carriers. If you do not see your insurance plan listed here, please give us a call. We are happy to go over coverage options with you.
Billing & Payment
We understand that billing in healthcare can be confusing.
While you are a patient at Ryse Health, your insurance(s) will be billed for each visit, whether in-person or telehealth. Ryse Health will also bill your insurance(s) for any remote patient care/data analysis that occurs between visits.
Depending on your insurance plan(s), your visits may incur a copay, coinsurance, or a balance owed towards your deductible. If you are responsible for any balance, you will receive electronic statements (text message and email statements) from our office.
Any unpaid balance you are responsible for can be paid in 3 ways:
- Online through the link in the e-statement you received (text message and email statements)
- You can call our office to pay over the phone
- You can pay in-person at your next appointment
Remember to always check with your insurance provider to confirm specific coverage and benefits for your visits.
What CPT codes are used to bill for care management services?
99490: Chronic Care Management, first 20 minutes
99439: Chronic Care Management, each additional 20 minutes
99487: Complex Chronic Care Management, first 60 minutes
99489: Complex Chronic Care Management, each additional 30 minutes
99426: Principal Care Management, first 30 minutes
99427: Principal Care Management, each additional 30 minutes
These codes are used to submit claims, and you will see them on your insurance EOBs and Ryse Health statements. While you don’t need to take any action with the codes, you can use them to call your insurance with any questions about your coverage.
Do care management services cost me anything?
Just like with traditional visits, that depends on your insurance coverage. For patients with traditional Medicare, care management services are covered under Part B, which means you may be responsible for a 20% coinsurance once your annual deductible has been met. If you have a Medigap plan or other supplemental insurance, it will typically cover the coinsurance.
For patients with Medicare Advantage plans or commercial insurance, coverage and costs vary. Typically, your specialist copay will apply to care management services once your annual deductible has been met. To determine how your specific insurance plan impacts your out-of-pocket cost, we recommend calling your insurance company and asking for coverage and cost details for the specific CPT codes.
